Hybrid Approach to a Right Aortic Arch With a Large Kommerell’s Diverticulum

A 60-year-old woman with a family history of aortic dissection in her mother was investigated for dysphagia. Her scan showed right arch large Kommerell’s diverticulum causing esophageal compression. left vertebral artery, which originated abnormally low the thorax, embolized preoperatively to limit operative difficulties. Two weeks later, thoracotomy allowed resection aneurysm an associated carotid-axillary bypass. Partial cardiopulmonary bypass moderate hypothermia beating-heart perfusion brain conducted. Five-year follow-up favorable outcome. multidisciplinary approach is safe and simple strategy complex cases.
